Abstract

Purpose: To evaluate the endothelial cell (EC) loss after insertion of corneal donor in Descemet stripping automated endothelial keratoplasty (DSAEK) using a new graft inserter, NS endo-inserter, which can be readily manipulated without collapse of anterior chamber during graft insertion.

Methods: Experimental study and prospective case series. Four experimental donor corneas and 18 patients undergoing DSAEK with a new graft inserter were included. Donor lenticles were inserted into an artificial anterior chamber with a new graft inserter and EC damage was examined by a vital dye assay using alizarin red S and trypan blue. EC count was assessed by specular microscopy 3 months after DSAEK in the clinical patients.

Results: The mean (± SD) EC loss after donor insertion with a new graft inserter was 7.2 ± 3.5 % (3.3 - 12.6%) in the experimental study. The mean pre- and postoperative EC density in the clinical patients were 2664 ± 158 and 2019 ± 602 cells / mm2, respectively. The mean EC loss 3 months after DSAEK was 24.4 ± 24.1 %.

Conclusions: Our newly developed NS endo-inserter may be a useful device for donor insertion in DSAEK.
