Abstract

Purpose: : To evaluate use of a femtosecond laser to make potentially stronger and more secure corneal transplant incisions.

Methods: : In this prospective, IRB–approved study, 6 eyes in 6 patients had stepped (tophat–shaped) incisions made in the cornea with the IntraLase femtosecond laser. The anterior incision was approximately 7mm diameter and the posterior approximately 9mm, with a connecting lamellar incision. The laser was used to cut a similar stepped incision in the donor tissue. The donor graft was secured with interrupted or running 10–0 nylon sutures.

Results: : Initial results showed all eyes had clear corneal grafts.

Conclusions: : The use of a stepped or contoured incision for penetrating keratoplasty was first reported over 40 years ago. However, this type of incision never gained widespread acceptance due to the difficulty of manually creating the matching incision configurations in the donor and recipient corneas. The femtosecond laser offers an efficient method to precisely create corneal incision configurations that may increase wound strength.
