Purpose: To evaluate visual outcomes of vitrectomy for epiretinal membrane removal in patients over 75 years of age. Methods: Retrospective review of vitrectomy for epiretinal membrane removal from May 1998 to August 2001 at The McPherson Retina Center at Baylor College of Medicine was performed. Preoperative visual acuity, duration of symptoms, time from diagnosis to surgery, preexisting macular edema, etiology of membrane; postoperative visual acuity, residual macular edema, and final visual acuity were evaluated and compared for patients younger or older than 75 years of age. Results: A total of 45 patients were evaluated; twelve in Group I (age 75 to 83 years), 33 in Group II (age 49 to 74 years). Thirty-four were idiopathic, 11 were secondary epiretinal membranes (eight had history of cryotherapy and/or scleral buckle procedure for retinal detachment, three had previous laser for retinal tear or branch retinal vein occlusion). Preoperative demography was similar for each group. Mean follow up was 14.7 months (3-39). Postoperative vision improved two lines or more in 75% (Group I) and 72.7% (Group II) patients. . There is no statistically significant difference (p=0.07). The average visual acuity improvement was 2.8 lines in Group I and 3.1 lines in Group II. There is no statistically significant difference (p=0.6). Final visual acuity was not related to duration of symptoms, time from diagnosis to surgery, nor preexisting macular edema. Poor visual outcome was related to residual macular edema. Retinal tear/detachment occurred in one patient. One patient lost 2 lines of best-corrected visual acuity due to cataract formation. Conclusion: Epiretinal membrane surgery is of equal benefit for patients over 75 years of age with visual outcome similar to those under 75. The decision for surgery should not be based on the patient's age alone.
