Abstract

Abstract: : Purpose: We investigated bacterial colonization and proliferation in anterior chamber with visco-elastic substance to pursue the factor of infectious postoperative endophthalmitis. Methods: Bacterial inocula were inoculated to the anterior chamber of Japanese white rabbit (27 eyes) after general anesthesia. Inocula were the following four groups, Group-A) Mixed inoculation of bacteria (MRSA MK99-3, about 100 CFU/eye) and visco-elastic substance (Healon, Pharmacia or Viscoat, Alcon)(14 eyes), Group-B) Separated inoculation of bacteria and visco-elastic substance (4 eyes), Group-C) Inoculation of bacteria after inoculation of visco-elastic substance (after 6, 12 or 24 hours)(6 eyes), Group-D) Inoculation of bacteria only (3 eyes). After clinical observation, bacterial culture of the aqueous humor that was gathered from removal eyeball after euthanasia was done. Histopathological examinations of the removal eyeball were performed. Results: Group-A showed symptoms of endophthalmitis though group-B, -C and -D did not. Bacterial culture of group-A showed remarkable bacterial proliferation (4.33 CFU/ml, log) in the aqueous humor in 24 hours post-inoculation though any bacteria were not detected after 48 hours post-inoculation. In other groups, any bacteria were not detected in the aqueous humor. Histopathological findings of group-A showed bacterial colonization to the iris after 48 hours post-inoculation. There were no differences between Healon and Viscoat about bacterial colonization and proliferation in this model. Conclusion: Visco-elastic substance raised the possibility of bacterial colonization and proliferation in anterior chamber, which might be result of protection from aqueous flow that removed bacteria from anterior chamber. There was few possibility of angle blockage effect of visco-elastic substance about bacterial colonization and proliferation. There is risk of infectious postoperative endophthalmitis from bacterial colonization and proliferation by immediate protection of visco-elastic substance.
