Abstract

Purpose : The aim of this study was to evaluate the effects of high intensity corneal cross-linking (CXL) with optimized beam profile setting performed with UV-X 2000 (AVEDRO, Massachusetts, USA) in comparison with a standard top-hat profile, based on corneal topographic and tomographic parameters.

Methods : 37 eyes of 37 patients with progressive keratoconus were treated between 2010 to 2013. The follow-up time was 36 month postoperatively. Group 1 included 18 eyes (age 31.7±9.9 years) which were treated with standard top-hat profile (TH). Group 2 included 19 eyes (age 33.5±11) which received a treatment with optimized beam profile (BO). The cross-linking was performed based on the modified Dresden protocol with a shorter irradiation time (10 min) and a higher energy (9 mW/cm2). The topographic and tomographic parameters were recorded by the Pentacam (OCULUS, Wetzlar, Germany).

Results : Keratometry at the apex (Kapex) decreased significantly from 56.46±6.86 D to 52.77±7.33 D (P=0.001) by TH-profile and from 53.73±4.44 D to 51.72±3.8 D (P=0.000) by BO profile three years after CXL. There was also a significant reduction at the flat (PTH=0.006, PBO=0.037) and steep (PTH=0.006, PBO=0.019) keratometry. The corneal thickness was significantly lower after the treatment and did not reached the baseline values (TH: -25.61±42.57 µm, P=0.005 and BO: -11.79±18.9 µm, P=0.017).

Conclusions : The corneal topography improved significantly after CXL in both groups, whereas the beam profiles did not distinguish from each other regarding the treatment effects.
