%X To measure the changes in mean gonioscopic angle width in phakic eyes six years following baseline evaluation in a population based cohort from rural South India. The Chennai Eye Diseases Incidence Survey (CEDIS) examined 2504 subjects, aged 45 years and older ,between 2007-2009 examined in the Chennai Glaucoma Study from 2001-2004. All subjects underwent a comprehensive glaucoma evaluation including IOP measurement and 4 mirror gonioscopy. Of these, 1894 participants who were phakic in the right eye at baseline and follow up were included. Angle width was graded based on the Shaffer grading(1-4) for each clock hour and the mean grade was computed. The mean baseline grade was subtracted from the mean follow up grade to calculate difference in angle width. Those with angle closure disease at baseline who had undergone laser peripheral iridotomy at baseline were analyzed separately. The mean age of participants at baseline was 49.5(SD: 8.9) yrs at baseline. There were 865 men and 1029 women. The mean angle grade at baseline was 3.12(SD: 0.38) and at follow up was 3.0(SD:0.6).The mean difference in angle width of the right eye among those with open angles at baseline was 0.16(SD: 0.6). More than 70% of eyes had no change in angle width as compared to baseline, 15% showed a decrease of 1 angle grade or more. Among the 122 eyes that had undergone a laser iridotomy at baseline the mean difference in angle width was 2.45(SD:0.9). Nineteen of 122 ( 85%) of angles were open post laser iridotomy. There were significant reductions in angle width in 15% of eyes with open angles over a 5 year period in a rural South Indian population.
