Abstract

Purpose: To compare the safety and visual outcomes of phakic intraocular lenses (PIOL) in a large myopic cohort.

Methods: This is a historical comparative cohort study. A total of 200 eyes of 114 myopic patients who were implanted with PIOL between June 2005 and December 2012 were included. Pre-operative, operative,and post-operative clinical data was collected.

Results: In our cohort 128 eyes were implanted with Visian implantable contact lens (ICL), and an Artisan iris-fixated phakic intraocular lens was implanted in 61 eyes. Other types of PIOLs were implanted in the remaining 11 eyes. Follow up had a mean of 24.4 months (range: 1-59). The mean preoperative spherical equivalent (SEQ) was −12.60±5.19, and −13.40±4.08 diopters (D) for ICL and Artisan(P=0.24),respectively. The preoperative uncorrected distance visual acuity (UDVA) was 20/200 or worse in 82.1% and 83.3% for ICL and Artisan, respectively. A postoperative UDVA better than 20/40 was achieved in 84.3% for ICL and 79.2% for Artisan at the last follow up. The mean postoperative SEQ was −1.15±1.9 and −1.90±1.09 D for ICL and Artisan, respectively (P<0.001). Intraocular pressure (IOP) at the last follow up was higher than baseline for both PIOL groups([17.5 vs 16 mmHg] p<0.001 for ICL) and ([17.4 vs 16mmHg] p= 0.005 for Artisan], however there was no statistical difference between groups(p=0.72).The rate of any complication was comparable in ICL implanted eyes compare to Artisan implanted eyes (19 [14.8%] eyes vs 9 [14.7%]; p value=0.98).The main complication of ICL was intraocular pressure elevation (9 [7%] vs 3 [4.9%] eyes in the Artisan group). Two (1.5%) eyes developed significant cataract and one (0.78%) eye had a rhegmatogenous retinal detachment in the ICL group.

Conclusions: ICL and Artisan showed comparable safety, predictability, and efficacy profile as PIOL in a large myopic cohort.
