Abstract

Purpose: To evaluate the reproducibility of Standardization of Uveitis Nomenclature (SUN) criteria for grading activity of intraocular inflammation.

Methods: Grading of 65 eyes of 41 patients with uveitis was conducted by one uveitis expert, one uveitis fellow and one resident who has been trained to measure the degree of activity according to SUN criteria. Agreement in grading anterior chamber (AC) cells and AC flare was calculated and measured using κ statistic. Demography features were also collected.

Results: In this cross-sectional study 22 men and 19 women with a mean age of 48.3 were studied . Bilateral uveitis was found in 23 patients and unilateral in 19 patients. According to the anatomical classification 16 patients had difuse uveitis, 14 anterior uveitis, 3 posterior uveitis and 3 intermediate uveitis. Five patients had no anatomical diagnosis. Out of 41 patients, 11 had Vogt-Koyanagi-Harada disease, 6 juvenile rheumatoid arthritis, 7 idiopathic uveitis, 3 Toxoplasmosis, 3 Fuchs uveitis syndrome, 2 herpes anterior uveitis, 2 acute anterior uveitis related to HLA B27, 2 toxicity, 1 Sympathetic Ophthalmia, 1 Sarcoidosis, 1 esclerouveitis, 1 Behcet syndrome and 1 acute retinal necrosis. The κ statistics for exact agreement on gradings of AC cells between the expert and the fellow was 0.45, between the fellow and the resident was 0.56 and between the expert and the resident was 0.62. The κ statistics for exact agreement on gradings flare between the expert and the fellow was 0.44.

Conclusions: The SUN criteria for grading AC cells and flare among physicians with different training degrees had a a moderate to substancial reproducibility.
