We compared outcomes and complications after DSAEK surgery in patients with or without a history of trabeculectomy.
Eighty-seven eyes of 74 individuals who underwent DSAEK at Yamaguchi University Hospital were enrolled in the study. The eyes were divided into two groups: those with a history of glaucoma treated with a previous trabeculectomy (TLE-DSAEK group), and those without a history of glaucoma surgery (DSAEK group).
Twenty-one eyes of 15 patients (11 men and 4 women, mean age ± SD of 69.1 ± 7.2 years, age range 55-84 years) were enrolled in the TLE-DSAEK group. Sixty-six eyes of 59 patients (26 men and 33 women, mean age ± SD of 74.3 ± 8.0 years, age range of 57-90 years) were enrolled in the DSAEK group. The mean follow-up time after DSAEK surgery was 17.0 ± 9.7 months in the TLE-DSAEK group and 21.1 ± 15.5 months in the DSAEK group. Kaplan-Meier survival curves for graft failure revealed no significant difference between the TLE-DSAEK and DSAEK groups. The mean endothelial cell density after surgery also did not differ significantly between the two groups. Ocular hypertension was manifest in three eyes (14.3%) in the TLE-DSAEK group and in nine eyes (13.6%) in the DSAEK group (P = 0.5446). Primary graft failure was apparent in one eye in each group (4.8% versus 1.5%, respectively, P = 0.3872). Detachment of the donor tissue developed in 4 eyes (19.0%) and 10 eyes (15.2%) with or without a history of trabeculectomy, respectively (P = 0.6721). Endothelial rejection occurred in two eyes (9.5%) and one eye (1.5%) in the TLE-DSAEK and DSAEK groups, respectively (P = 0.0798).
Outcome and postoperative complications of DSAEK did not appear to be affected by previous trabeculectomy.
