To determine if subretinal drusenoid deposits (SDDs) can be reliably detected on widefield en face slab images derived from spectral domain (SD) and swept source (SS) optical coherence tomography (OCT) volume scans.
Patients with the diagnosis of dry AMD were imaged using a commercial SD-OCT instrument (Cirrus HD-OCT, Carl Zeiss Meditec Inc., Dublin, CA) with a central wavelength of 840 nm and a prototype Zeiss 100kHz SS-OCT instrument (Carl Zeiss Meditec Inc., Dublin, CA) with a central wavelength of 1050nm. Seven different en face slabs were evaluated with thicknesses ranging from 20 to 55 microns and positioned at distances up to 55µm above the retinal pigment epithelium (RPE). Multiple overlapping 6mmX6mm SD-OCT en face images of the posterior pole were assembled as a montage from a single patient and compared with a single 9mmX12mm SS-OCT en face slab image. These widefield en face images were then compared with color, autofluorescence, and infrared reflectance images.
A total of 160 patients (256 eyes) were scanned with both OCT instruments, and 57 patients (95 eyes) also underwent multimodal fundus imaging. Of 95 eyes, 32 (34%) were diagnosed with reticular pseudodrusen (RPD) using multimodal imaging. All of these eyes with RPD demonstrated a pattern of SDDs on widefield en face OCT imaging similar to the pattern observed for RPD. The en face slab image that consistently identified SDDs was the 20 micron thick slab with boundaries from 35 to 55 microns above the RPE.
Widefield en face slab imaging with SD-OCT and SS-OCT can reliably detect SDDs and could replace multimodal imaging for the diagnosis of RPD.
