To investigate the changes in in vivo anterior segment anatomy, using swept-source optical coherence tomography (SS-OCT) in response to uncomplicated cataract extraction and posterior chamber intraocular lens (IOL) implantation in cataract patients with and without primary open-angle glaucoma (POAG).
In this prospective study, each participant underwent SS-OCT (Casia SS-1000 OCT; Tomey, Inc., Nagoya, Japan) to obtain 128 volumetric high-resolution radial B-scans centered on the pupil center and a complete ophthalmologic examination. Pre- and postoperatively, anterior to the scleral spur angle opening distance 500 µm (AOD500), the trabecular-iris space area (TISA500), the trabecular-iris angle (TIA500), anterior chamber depth (ACD), lens vault (LV), anterior chamber width (ACW), and total, superior, inferior, nasal, and temporal quadrant anterior chamber volume (ACV) were obtained using the SS-OCT’s built-in software. Intraocular pressure (IOP) was recorded.
Twenty-six eyes of 23 patients (8 POAG eyes and 18 control eyes) were included. There was no difference in preoperative characteristics between the POAG and control groups (all P >0.05, Table 1). After cataract extraction, AOD500, TISA500, TIA500, ACD, LV and ACV (total, superior, inferior, nasal, and temporal quadrant) increased significantly in the POAG and control groups (P<0.05, Table2). Postoperative IOP decreased significantly in the control group (P<0.0001). However, in POAG group, the IOP reduction was not statistically significant (P=0.155). There was no difference in IOP changes or anterior ocular biometry changes after cataract extraction between the POAG and control groups (all P >0.05).
The anterior chamber angle widened and depth and volume increased significantly,after cataract extraction and IOL implantation in both normal and POAG eyes.
