To report the mid-term result of non-strippinng Descemet's stripping endothelial keratoplasty (nDSAEK) for endothelial dysfunction.
Twenty eyes of 20 patients (10 men and 10 women) with cornea edema that underwent nDSAEK were retrospectively analyzed. Pre-cut donor buttons prepared by US eye banks were inserted through a 4.1mm limbal incision by the pull-through technique using the Busin glide. Air was injected to the anterior chamber at the end of surgery and removed after 2 hours. All cases were operated by the same surgeon (S. S.) at the Keio University Hospital or Miyata Ganka Clinic. We investigated visual acuity, spherical equivalent (SE) and endothelial cell density up to 6 months in all cases.
Mean preoperative best-corrected visual acuity was 0.66±0.52 logMAR, and postoperatively, 0.19±0.18 logMAR at one month, 0.13±0.21 logMAR at 3 months, and 0.11±0.17 logMAR at 6 months. There was significant improvement in vision (p<0.05). Mean postoperative SE was -0.41±2.11D preoperatively, -0.44±1.33D at a month, -0.72±2.21D at 3 months, and -0.81±2.07D at 6 months after nDSAEK showing a slight myopic shift which was not statistically significant. Mean loss of donor endothelial cell density was 10.4% at a month, 11.1% at 3 months, and 13.2% at 6 months.
This retrospective study shows that nDSAEK offers significant improvement in vision with no significant refractive change and mild endothelial cell loss.
