Abstract

Purpose: : to report the SD-OCT features in Dome Shaped Maculopathy associated with neuroepithelium detachment.

Methods: : patients diagnosed as having a Dome Shaped Maculopathy associated with a neuroepithelium detachment. Inclusion criteria were a dome Shaped Macula within a tipe 1 or 2 of myopic staphiloma on SD-OCT ( Spectralis HRA + OCT, Heidelberg Engineering , Heidelberg, Germany ) with neuroepithelium detachment in the macular area . Visual acuity and OCT findings were collected and reviewed.

Results: : twenty eyes of 17 consecutive patients ( 2 males 15 females ), mean age 57 years ( range 32 - 79 ), mean refraction -7,25 sph ( range -1,75/-13,75 sph ), mean BCVA ( 20/40 ETDRS charts ) were included in the analysis. 20 eyes showed hyper-reflective spots within the neurosensory retina. The external limiting membrane was preserved in all but 1 patient . 19 eyes ( 95% ) showed defects at the level of the retinal pigmented epithelium . All eyes showed hyper-reflective material within the neurosensorial detachment on the outer surface of the elevated neurosensory retina possibly corresponding to photoreceptors outer segment elongation. Three ( 15% ) eyes also presented hyper-reflective mounds at the level of Bruch membrane/RPE complex partially shadowing the underlying choroid. The mean VA between patients having or not this accumulation was not significally different ( Wilcoxon Sum Rank Test, p 0,7 ). The IS/OS junction was interrupted in 15 eyes ( 75% ).The mean VA between patients with a continuous or interrupted IS/OS junction was not significally different ( Wilcoxon Sum Rank Test, p 0,2 ) .One patient had an epiretinal membrane without wrinkling . No signs of vitreous traction were detected.

Conclusions: : SD-OCT findings in Dome Shaped Maculopathy associated with neuroepithelium detachment are similar to those found in central serous chorioretinopathy. Altought there are no data in the literature concerning the natural history, these findings are compatible with a chronic disease. Vitreo-macular tractions do not seem to be involved in the pathogenesis of the neuroepithelium detachment.
