Abstract
Purpose: :
To evaluate the safety and efficacy of combined cataract surgery and Glaukos iStent implantation for coexistent primary open angle glaucoma and cataract
Methods: :
Prospective, noncomparative, uncontrolled, non-randomised, interventional study. Subjects older than 18 years old with uncontrolled mild or moderate primary open-angle glaucoma (including PSX and pigmentary) and cataract underwent phacoemulsification, intraocular lens implantation with ab-interno gonioscopically guided implantation of one Glaukos iStent. Visual acuity, intraocular pressure (IOP), glaucoma medications and endothelial cell count were assessed in patients with a minimum follow-up of 3 years.
Results: :
19 patients aged 58 to 88 years (mean age 74.6±8.44years) were included. Mean follow up was 53.68±9.26 months. Mean IOP was reduced from 19.42±1.89 mmHg to 16.26±4.23 mmHg (P=0.002), that means a 16.27% decrease in the IOP. The mean number of ocular hypotensive medications decreased from 1.26±0.45 to 0.79± 0.92 (P=0.05). The mean visual acuity significantly improved from 0.29±0.13 to 0.62±0.3 (P<0.001). Mean endothelial cell count decreased from 2447.83±243.81 to 1875.18± 415.68 cells/mm2 (P<0.001). 47% of the patients were able to discontinue all glaucoma medications at the end of the follow-up.
Conclusions: :
The combination of cataract surgery and Glaukos iStent implantation seems to be and effective and safety surgical approach in coexistent open angle glaucoma and cataract patients.
