Purpose: : During vitrectomy in phakic patients lens damage can occur during shaving of the peripheral vitreous gel. Modifying the vitrector to have a slight bend can diminish this risk. This study evaluates the cutting and aspirating characteristics of a straight versus bent 25-gauge vitrector in vitro.

Methods: : A 25-gauge vitrector was modified by bending the shaft midway along its length against the curvature of a 20D lens. The angle of the bent vitrector measured 14 degrees. Vitreous from an enucleated porcine eye was removed and 1 cc placed in a standard syringe. The straight cutter (settings: vacuum 200, cut rate 1800) was then placed in the syringe and time to complete removal was recorded. The same procedure was then done with the bent cutter on the same settings. The aspiration rate (cutter off, vacuum 200) of each type of vitrector was then tested by placing 2 cc of balanced salt solution (BSS) in a syringe and recording time to removal. Each type of vitrector was tested five times with both vitreous and BSS. Data was analyzed using a paired two-tailed t-test for comparison of two small sample means.

Results: : The average time for removal of vitreous with the straight cutter was 19.32 seconds and for the bent cutter 19.26 seconds. There was no statistically significant difference between the removal rates of the straight versus the bent cutter (p-value = 0.87). The average time for aspiration of BSS with the straight tip was 14.3 seconds and for the bent tip 14.16 seconds. There was no statistically significant difference between the aspiration rates of the straight versus the bent cutter (p-value = 0.55).

Conclusions: : Straight vitrectors run the risk of intraoperatively damaging the crystalline lens when removing the anterior vitreous. A bent vitrector is shaped to match the contour of the lens to avoid contact while still allowing efficient removal of the vitreous base. Both types demonstrate an equally efficient rate of removal of both vitreous and balanced salt solution in vitro.
