To investigate intraretinal changes after a successfully cataract surgery in patients with previous pars plana vitrectomy and indocyanine green-assisted peeling of a macular pucker and the internal limiting membrane (ILM).
Fifteen eyes of 15 patients with previous 23 gauge pars plana vitrectomy and ILM peeling scheduled to undergo cataract surgery by phacoemulsification and intraocular lens implantation. All eyes were imaged using Spectralis OCT (Heidelberg Engineering©) using the AutoRescan function which allows follow-up scans in precisely the same location as the baseline scan. Spectralis OCT scans, infrared fundus imaging and biomicroscopy were performed at baseline, week 1, month 1 and 3 after surgery. OCT raster scans were further analyzed regarding the area of retinal thickening. All planimetric measurements were performed using the program ImageJ © (National Institutes of Health).
The mean (±SD) age of patients was 68 (±5) years (range 56-77).The mean visual acuity score was 0.3 (±0.13 SD) Snellen lines improved significantly to 0.83 (±0.15 SD) Snellen lines (p<0.001) three months after cataract surgery. Mean central millimeter thickness (CMMT) was 402 (±45µm SD) at baseline and increased to 417 (±48µm SD) at month 3 (p=0.001). Seven patients were developed increase of CMMT more than 30µm one month after cataract surgery.Three months after surgery, the area of swelling was significantly bigger in most patients compared to baseline values.
This is the first study showing that patients with previous pars plana vitrectomy and ILM membrane peeling are more likely to develop macular edema after cataract surgery.
