Abstract

Purpose: : To report the visual and clinical outcomes of subthreshold 577-nm micro-pulsed laser photocoagulation (SMLP) technique for chronic idiopathic central serous chorioretinopathy (ICSC) with focal foveal leakage or chronic diffuse leakage with foveal involvement.

Methods: : Nine eyes with ICSC with duration longer than 4 months with focal foveal leakage or with diffuse leakage involving the fovea were treated. High density focal laser were delivered targeting the angiographic leakage points (including the central foveal area) as well as the adjacent normal retina. Main outcomes were change in ETDRS best correct visual acuity and central macular thickness measured using optical coherence tomography. Patients were followed 1, 2, 3, and 6-months after treatment. Pre and post-treatment fluorescein angiography (FA) and microperimetry were assessed.

Results: : No visible clinical signs of treatment could be detected either on fundus examination or FA. Microperimetry examination showed no induced laser-related focal damage to the treated area. Improvement of 3 or more lines on the ETDRS chart was achieved in 6 (67%) of the patients. At the last follow-up visit, subretinal fluid were completely resolved in 8 (89%) eyes. One third of the eyes (3 eyes) needed to be re-treated.

Conclusions: : This selective SMLP therapeutic technique using the 577 nm yellow laser applied to foveal leakage with extension to diffuse affected as well as normal retina areas enable a selective and optimized approach, providing a possible more comprehensive laser therapeutic strategy for the treatment of ICSC patients with foveal leakage, with no laser-induced damage.
