Abstract

Purpose: : To conduct retinal and Outer Nuclear Layer (ONL) volumetric and average thickness analysis in normal and dry Age-Related Macular Degeneration (ARMD) eyes.

Methods: : Six dry ARMD and 6 normal eyes (average age 67) were in our study. Study eyes did not have neovascular changes, ocular injection/surgery/laser (excluding cataract), other macular diagnoses, or spherical equivalent outside +5.00 to -6.00 diopters. Visual acuity, AREDS class (AREDS: design implications, Control Clin Trials, 1999), and Cirrus HD-OCT (Carl Zeiss Meditec, Dublin, CA) with macular cube 512x128 were noted. ONL measurements were done after remapping lines to the external limiting membrane and outer plexiform layers for each frame. Drusen area was calculated using "Slab view" (2D plane). Volume and thickness were plotted against acuity, AREDS class and drusen count/area.

Results: : ONL thickness was greatest at the central foveal region in normals (90±14 µm) and dry ARMD eyes (84±21 µm). Normal eyes had larger ONL/total retinal volume and average retinal/ONL thickness, compared to dry ARMD eyes (not statistically significant). Average ONL thickness had a negative correlation with drusen count (R2= 0.657) and drusen area (R2= 0.553). ONL volume was negatively correlated with drusen count (R2= 0.593) and drusen area (R2= 0.532). Average retinal and ONL thickness both had positive correlations with total retinal volume with R2= 0.991 and 0.975, respectively. Drusen area had high correlation with AREDS class (R2= 0.854).

Conclusions: : This is the first study that measured ONL volume in normal and dry ARMD eyes. ONL volumes were not significantly different in normal compared to dry ARMD eyes. Drusen area was a novel approach to analyze drusen and it correlated well to AREDS classification.
