Abstract

Purpose: : Show the efficiency of a new self-made conformator, in order to prevent synechiae after endoscopic dacryocystorhinostomy ( DCR).

Methods: : prospective study concerning 20 endoscopic DCR in 2008, carried out with the same technique and the same team of operators : local anesthesia by contact - Ablation of the nasal mucosa - osteotomy compared to the lacrymal sac - wide opening of the lacrymal sac mucosa - Bicanaliculonasal catheterization (IBCN) - Insertion of a 1mm thick "hockey stick" shaped conformator made of silicone below and in front of the stomy, of a size adapted to the endonasal architecture - sutured to the septum. The conformator is removed at J10-J12.An endoscopic control is carried out 1 and 2 months later with ablation of the IBCN. We note the possible appearance of synechiae before the rhinostomies.

Results: : No synechiae is found during these 3 clinical controls.

Conclusions: : The use of a self-made silicone conformator before the rhinostomy while performing a DCR, is a simple and cheap technique, which facilitates postoperative care, prevents the formation of synechiae and avoids partial medium turbinectomy recommended by certain authors.
