Macular Hole Recurrence After Cataract Extraction

Purpose: : The purpose of this study is to evaluate the frequency of macular hole recurrence after cataract extraction.

Methods: : This study is a retrospective consecutive case series. Forty-four eyes with primary macular holes closed by vitrectomy were evaluated. All eyes had cataract extraction with standard phaco techniques between one and 126 months (mean 17.1 months) after vitrectomy. The mean follow-up was 62.2 months (range, 9 to 181 months). Several variables were evaluated in relationship to the main outcome measure, macular hole recurrence.

Results: : Three macular holes re-opened (6.8%) with a mean follow-up 86.7 months (range, 71 to 98 months). There were five eyes with cystoid macular edema (CME) in the post operative period after cataract extraction. None of the five post-cataract CME eyes developed macular hole recurrence. The difference between the rates of recurrence between CME and non-CME groups was not statistically significant (p = 0.58).

Conclusions: : We conclude that cataract extraction after successful vitrectomy for macular hole does not increase the rate of re-opening. While some previous reports have suggested pseudo-phakic cystoid macular edema represents a significant risk factor for macular hole recurrence, we did not observe an association in this cohort.
