To assess biocompatibility and stability of novel biodegradable "heavier than water" vitreous substitutes designed as potential drug release systems.
Vitreous substitution was performed in 6 rabbits in accordance with ARVO’s Statement for the Use of Animals in Ophthalmic and Vision Research. Control polymer was a non-biodegradable EC approved PFCL (HPF10, GraftTec, Abeamed Inc, Coral Gables, FL); biodegradable polymers (Al.Chi.Mi.A, SLR, Italy) were ALC5 (d=1.33g/ml, n=1.3385), FL14 (d=1.34g/ml, n=1.3450), GE68 (d=1.35g/ml, n=1.3430) and BM61 (d=1.34g/ml, n=1.3390). Tonometry and pachymetry readings were taken preop and at all follow-ups. Baseline slit-lamp exam were done in both eyes. After a 5mm wide conjunctival limbal dissection, the conjunctiva/tenon was delaminated and, using a fundus contact lens, a 23ga needle track was made 4mm past the limbus, ~0.5cc of vitreous was extracted and the track closed with a 8-0 vicryl mattress, ~0.5cc of polymer was injected via a 27ga cannula positioned above the retina, the mattress tightened and the conjunctiva closed with 10-0 nylon. The rabbits were examined at POD 1, 3, 7, 14, 22, 28, 35 and 50 under the operation microscope. Photo slit-lamp, RetCAM and OCT imaging was performed periodically.
There were no clinical signs of infection or inflammation and pachymetry and intraocular pressure remained normal in all rabbits throughout the study. Lens opacities slowly developed with time in all animals except FL14 and GE68. The retina remained normal in all polymers except BM61. Polymer emulsification together with minute floaters (<10um) in the vitreous were observed as early as POD 5 in the HPF10 control and at POD 14 with test polymers except ALC5 where a single, albeit smaller, bubble remained.
