UHR-OCT for Higher Definition of Early Retinal Changes After Intravitreal Bevacizumab (Avastin®) Application in Patients With AMD

Abstract

Purpose:: To investigate early structural changes of the Retinal/RPE/Choroicapillaris complex before and after intravitreal application of bevacizumab(Avastin®) in patients with AMD by comparison of standard Stratus OCT( Carl Zeiss Meditech) and UHR-OCT imaging systems.

Methods:: Six patients with neovascular AMD were examined with UHR-OCT and Stratus OCT one day before and 10-14 days after intravitreal application of 1.25mg (0.125ml) bevacizumab (Avastin®). UHR-OCT examinations were performed using a commercially available ultra-broad bandwidth (160 nm) Titanium:sapphire based femtosecond laser system with a high axial resolution of 3 µm and a transverse resolution of ~15 µm. Each data set produced a volume of 60 tomograms. Standard Stratus OCT(Carl Zeiss Meditec) images were performed simultaneously. Central Retinal Thickness(CRT) was measured with the retinal mapping programm of StratusOCT. For UHR-OCT a recently of our group developed measuring system was used. In addition to clinical examination BCVA testing was performed using EDTRS and Radner-charts.

Results:: 6 Patients,3 male and 3 female, age 74 to 86 years(average 78,8),2 right, 4 left eyes with active CNV or vascularised PED were included. CRT decreased significantly in 3 cases in Stratus OCT retinal mapping(min-128µm,max-519µm); average decrease of CRT was 132µm or 21.1% after injection. UHR-OCT measuring system showed a decrease of CRT in all cases(min-21µm,max-347µm), average decrease of CRT was 113µm or 15.1%. Effects of bevacizumab(Avastin®) on the single different layers could be shown with higher definition using UHR-OCT.In 3 cases residual fluid was only visible with UHR-OCT. Distance VA improved in 4 of 6 patients(min+6,max.+37 EDTRS letters), 2 patients showed stabilisation in distance VA(-1,-2 letters). All patients showed improvement of reading VA(min.+0.1 logRAD,max.+0.5 logRAD).

Conclusions:: Unprecedented resolution of the UHR-OCT provides more detailed information of early structural changes in the Retinal/RPE complex after intravitreal antiVEGF application. Better visualisation of the efficacy of new drugs will help in the decision of further treatment and follow up.
