Abstract

Purpose:: To assess efficacy, safety, stability, and predictability of wavefront-guided laser epithelial keratomileusis (LASEK) in myopic patients with the Bausch and Lomb Technolas 217 Zyoptix 100HZ system.

Methods:: In this retrospective study, we analyzed the results of 72 eyes of 48 patients who underwent custom LASEK. All treatments were performed by the same surgeon using the Bausch and Lomb Technolas 217 100HZ excimer laser. Primary outcome variables included uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), and manifest refractions measured at 1 day, 1 week, 1 month and 3 month post operatively. Data on UCVA, BCVA, efficacy, safety, stability, and predictability were analyzed.

Results:: Preoperatively mean sphere was -5.64 +/- 1.84 D (range -1.25 to -7.75), mean cylinder was + 0.70 +/- 0.64 D (range pl to +2.75), and mean spherical equivalent was -4.27 +/- 1.79 D (range -0.88 to -7.13). Preoperative mean BSCVA (in decimal equivalent) was 0.99 (20/20) +/- .003. The mean postoperative BSCVA at 3 months was 1.01 (20/20) +/-.02. The ratio of postoperative BSCVA to preoperative BSCVA (safety) was 1.01 (20/20) +/- 0.11. At three months, no eyes lost >or = 2 lines of BSCVA. Four eyes gained one line of BSCVA. The mean postoperative UCVA at three months was 0.95 (20/20) +/-0.19. Uncorrected visual acuity was 20/40 or better in 98% of eyes and 20/20 or better in 74% of eyes. The ratio of postoperative UCVA to preoperative BSCVA (efficacy) was 0.95 (20/20) +/- 0.19. The mean subjective manifest spherical equivalent at three months was -0.04 +/- 0.54. It was within 1.00D of emmetropia in 98.1% of eyes and within 0.5 D of emmetropia in 75% of eyes. There was an average change of 0.42 +/- 0.34 in spherical equivalents between the 1 month and 3 month postoperative visits.

Conclusions:: Wavefront guided LASEK in myopic patients with the Technolas 217 100HZ Excimer Laser is an effective, safe, predictable, and stable procedure. Further studies are warranted to determine the long term effects of this procedure.
