Purpose: To determine the morphological changes of the retina of patients with unilateral acute zonal occult outer retinopathy (AZOOR). Methods: Ten patients (4 men and 6 women; median age 31 yrs, range 25–64 yrs) with unilateral AZOOR were studied. The fundi of all patients appeared normal with normal fluorescein angiograms. Optical coherence tomography 3 (OCT3) was used to scan the fovea with 5mm vertical and horizontal scans. The thicknesses of the retina and nerve fiber layer in macula of both affected and fellow eyes were measured manually. The measured points were fovea, 1 mm and 2 mm superior, inferior, temporal and inferior from the fovea. The results from the affected and fellow eyes were compared. Results: The average retinal thickness of the affected eyes relative to the fellow eyes were; 74.9% at the fovea, 87.1% at 1 mm superior, 90.4% at 2 mm superior, 85.5%, at 1 mm inferior, 93.9% 2 mm inferior, 86.6% at 1mm temporal, 89.2% at 2 mm temporal, 85.0% at 1 mm nasal, and 90.1% at 2 mm nasal from the fovea. The macular thickness of the affected eyes were significantly thinner than that of the fellow eyes at all points (P<0.05) except for 2 mm inferior of the fovea. The thickness of the retinal nerve fiber layer in the affected eyes was not significantly different from that of the fellow eyes. Conclusions:These results demonstrated that there are significant morphological changes in the macula of AZOOR, even though there are no visible fundus abnormalities.
