Purpose: Ocular decompression retinopathy is a rare complicationof filtration surgery and is manifested by retinal hemorrhagesone or more days following a dramatic reduction in intraocularpressure. It is thought that visual decline is caused by macularhemorrhage, and visual return is variable. In this study, opticalcoherence tomographic (OCT) findings of one such case are presented.Methods: A 47 year–old woman was referred for vitreoretinalevaluation of retinal hemorrhages following uncomplicated trabeculectomyfor glaucoma secondary to herpetic uveitis. Serial color fundusphotographs and OCT3 scans through the macula were obtained.Results: Preoperatively, her visual acuity was 20/25 and herIOP on maximal medical therapy was 35–40 mm Hg. On thefirst postoperative day, her vision was 20/70, her IOP was 8mm Hg, and numerous intraretinal hemorrhages had developed.Vision decreased two days later to 20/400. and areas of hemorrhagerevealed hyperreflectivity consistent with subhyaloid and intraretinalblood, and several, large, clear foveal cysts (Figure 1). Treatmentwith topical nonsteroidal anti–inflammatory (NSAID) medicationfor two weeks resulted in visual improvement to 20/40 and areduction in foveal swelling (Figure 2).Conclusions: Hemorrhages in decompression retinopathy followingfiltration surgery may occur in various retinal layers. Visualdecline may be due to cystic macular edema, which may be amenableto topical treatment.
