Abstract: : Purpose: To assess the effect of intravitreal triamcinolone injection at the time of cataract surgery on the treatment, and the prevention of the exacerbation, of diabetic macular oedema Method: Longitudinal pilot study of 10 patients (16 eyes) undergoing cataract surgery with phacoemulsification and IOL. Patients either had persistent foveal oedema despite previous laser or foveal oedema with no adequate view to perform pre–operative laser. All patients had 4 mg of triamcinolone injected via the inferotemporal pars plana into the inferior vitreous cavity at the end of surgery Results: Pre–operative visual acuities ranged from 20/120 to HM. Eight patients ( 14 eyes) had clinically dry maculae at six weeks following surgery and all had a subjective improvement in visual acuity. There was an objective improvement in vision in 14 eyes with a median improvement of 3.4 lines. 50% of eyes achieved 20/40 vision or better which compares favourably with published studies of similar cases. Follow up ranged from 3–15 months with no deterioration in vision observed. There were no cases of endophthalmitis. Two cases developed transiently raised intraocular pressure which was controlled with topical medication. Post operative macular laser was applied in 7 eyes. Conclusion: These results are encouraging with an improvement in vision in most cases and no deterioration in macular oedema observed. The technique deserves further study.
