Purpose: To describe the clinical features, management and outcome of a patient who presented an intraocular parasite. Methods: A thirty year–old female farmer who complained of foreign body sensation, red eye and blurred vision for two weeks. Results: We found a visual capacity of 20/300 and 20/20 right and left eye, respectively. Right eye biomicroscopy revealed an hyperemic bulbar conjunctiva and ciliary inyection +++, anterior chamber celularity +++/++++, endothelial pigment clusters and posterior synequiae in meridian IV. Funduscopy revealed an intraocular linear shape motile parasite. There were also diffuse areas of retinal hemorrhage, irregular folds in the Retinal Pigmented Epithelium and a macular hole. There were no pathological findings in the left eye. B–mode ultrasonography was performed in the right eye showing vitreous condensations and a motile foreign body measuring 1.32 x 1.37mm with a partial posterior vitreous detachment. Intraocular myasis was diagnosed. Conclusions: We performed a vitrectomy and parasite extraction, and the larvae was identified as an Alaria Mesocercaria.
