Abstract

Abstract: : Purpose: The retinal mapping software of optical coherence tomography (OCT) allows measurement of retinal thickness in the posterior pole. Retinal thickness in the macular area after macular hole surgery was evaluated by OCT. Methods: In a prospective study, 19 eyes of 19 patients were enrolled in the study. All eyes were treated with a standard three-port pars plana vitrectomy with surgically induced posterior vitreous detachment, peeling of the internal limiting membrane (ILM) after staining with indocyanine green (ICG) dye, and injection of diluted sulfur hexafluoride gas. OCT measurements of retinal thickness in the macular area were taken preoperatively, and at 1, 3 and 6 months postoperatively. Six radial scans, each 6mm long, centered on the patient's fixation point were used for OCT measurements. Two-dimensional color-coded maps were also obtained. The posterior pole was divided into 3 areas: Area A, area covering 1000 µm diameter from the fixation point; Area B, area covering 1000 µm from the edge of Area A; and Area C, area covering 1500 µm from the edge of Area B. Results: As expected, retinal thickness measurements in Areas A and B were decreased within the first postoperative month. Furthermore, retinal thickness measurements in the same areas at 1 to 3 months postoperatively showed a continuous decreasing trend. However, retinal thickness measurements in the Areas A and B between 3 to 6 months postoperatively were not statistically significant. Retinal thickness measurements in both areas at the 1-month postoperative period had the greatest statistical significance compared with the other periods. Retinal thickness measurements in Area C did not differ from normal values. Conclusion: In eyes with macular holes, OCT measurements in the entire macula showed retinal thickening preoperatively. Postoperatively, the decrease of retinal thickness was greatest at the 1-month period and continued until the 3-month postoperative period, suggesting that anatomical restoration of macula can be achieved within 3 months after surgery.
