Purpose: Indocyanine green (ICG) die is useful for staining the anterior capsule during difficult capsulohexis in mature cataract and for staining the retinal internal limiting membrane during vitrectomy surgery. However, ICG absorption has been recognized in hydrophilic intraocular lenses (IOLs). In this study, we analyzed the absorption and diffusion of ICG in hydrophilic IOL. Methods: First, a hydrophilic hydrogel IOL (H60M, Bausch & Lomb) was prepared and soaked in ICG solution. After incubation, the IOLs were washed with physiological saline and IOL images were stored using a digital camera and scanning laser ophthalmoscope (SLO, Rodenstock). Diffusion of ICG from the hydrogel IOL into saline was also observed. Second, the hydrogel IOL was soaked in ICG solution and the IOL was then implanted in an albino rabbit after cataract operation. Postoperatively, the aqueous humor and the IOL were observed using SLO to detect ICG. Results: The hydrogel IOL was strongly stained with ICG after a 60 minute incubation and was destained after 24 hours in physiological saline, but ICG was detected by SLO as long as 3 weeks later. The ICG diffusing into the aqueous humor was detected using SLO after IOL implantation. Conclusions: ICG dye can penetrate and remain in hydrogel IOLs. The hydrogel IOL is a potential new drug delivery system after cataract surgery. Further research is warranted.
