Abstract: :
Purpose:The purpose of this study was to evaluate the effect of 5 fluorouracil (5FU) subconjunctival (SC) infiltration in pterygium prior to excision surgery. Methods:A prospective study including 53 pterygia (52 patients) was performed. 52,8% were primary and 47,2% were recurrent lesions. Patients were evaluated according to age, gender, symptoms and aspect of the lesion. They were divided randomly in two groups (G): G1 (25 cases) patients submitted to 5FU SC injection 30 days pre-operative, and G2 (28 cases), 5 FU SC injection 10 days pre-operative. All surgeries were performed by the same surgeon, using the same surgical technique. The patients were evaluated at 7, 30 and 60 postoperative days (PO) for complication and recurrence rate identification. The results were submitted to statistical analysis. Results:G1 patients (17 primary and 8 recurrent pterygia) showed an average age of 48 years-old, male prevalence and recurrence rate of 52,9% for primary and 75,0% for recurrent pterygia. G2 (11 primary and 17 recurrent) presented an average age of 49 years-old, female prevalence and recurrence rate of 27,3% for primary and 47,1% for recurrent pterygia. Conjunctival hyperemia (44,0% in G1 and 50,0% in G2 at 60 PO), dehiscence (56,0% in G1 and 10,8% in G2 at 7 PO), 1 keratites and 1 conjunctival cyst were the complications observed. Irreversible alterations due to drug SC infiltration toxicity did not occurred. Conclusion:Deletery effects to the eye were not observed with the subconjunctival infiltration of 5FU before the pterygium excision. Recurrence rate was higher among recurrent pterygia and in patients who received the drug 30 days before surgery, which may be due to the transient antifibroblastic effect of 5 FU.
