Purpose: To test fundus non-mydriatic digital image as a future instrument (virtual ophtalmologist) to detect non-proliferative diabetic retinopathy Method: We did a first ophthalmologic exam to all the patients between 20-64 years old who never has had an ophthalmologic exam and with risk factors as : family diabetes, indian american family or new diabetics. We took pictures of the fundus retina with a mydriatic camera TRC-NW6S Topcon. We select the best pictures (100% quality) to make a mosaic of 5 pictures of each eye. One hundred patients were selected within our criteria since sept 2000. Each patient was follow up every 4 months for pictures and mosaic-ensemble with the 100% pictures quality selected by a technician in ophthalmic photography. The mosaics were sent to ophthalmologist who in the confortability of his schedule could reviwed the pictures without seen the patient. Each patient continues to see his regular ophthalmologist who was out of the study Results: Ten out of one hundred patients were detected with non-proliferative early diabetic retinopathy with the mosaic pictures, by no need of ophthalmologist exam. Up to now any of the others ninety patients has been detected with the disease by another method. Non-mydriatic mosaic pictures method has 100% sensibility. Conclusions: Virtual ophthalmologist is a concept that can give us permanent charts, very low cost eficacity, 100% suceptibility
