Abstract

Abstract: : Purpose:To determine the incidence of RD following Nd:YAG for secondary cataract Methods:529 consecutive eyes of 465 patients which underwent Nd:YAG because of visually disturbing secondary cataract were followed prospectively for up to 49 months (median 19 months) in order to determine the incidence of RD. Prior to Nd:YAG all eyes had undergone uncomplicated phacoemulsification with implantation of a posterior chamber intraocular lens (PH+PC/IOL). Eyes with additional surgical procedures or panretinal laser coagulation were excluded but eyes in which a retinal hole had been treated prior to Nd:YAG were included (5 eyes).The fundus was examined by binocular ophthalmoscopy in full mydriasis. Results:142 male and 323 female patients with a median age of 78 years were examined. The axial length of the eyes ranged from 20.5 mm to 31.3 mm, with a median of 23.3 mm. The incidence of RD at six months was 0/482 eyes (0%, 95% CI: 0 - 0.8%) at 12 months 0/384 (0%, 95% CI: 0 - 1.0%) at 24 months 1/ 197 (0.5%, 95% CI: 0 - 2.8%). The only RD developed in an eye out of a horse-shoe tear which had developed one year after PH+PC/IOL and was treated by argon laser coagulation. This was six years prior to Nd :YAG. If one excluded the 5 eyes with preexisting retinal holes the incidence at 24 months is 0/196 (0%, 95% CI: 0 - 1.9%). Later on no other eye developed RD. Conclusion:RD is a rare complication of Nd:YAG. The incidence of RD is lower in this study than that reported in the majority of earlier studies. We believe that the decline in incidence is most likely due to advances in cataract surgical technique.
