To longitudinally document perivascular sheathing during the course of retinal vasculitis of various origins by infrared flood illumination adaptive optics (FIAO) imaging.
Seventeen eyes of 17 patients with uveitis of various etiologies associated with venous sheathing underwent FIAO imaging (rtx1, ImagingEye, Orsay, France). Images were quantitatively analyzed by semi-automatic segmentation to document change over time of the width of perivenous opacification and of vessel diameter.
At initial examination, the mean width (±SD) of perivenous opacification was 89.6µm ± 54.9. In 10 cases, the underlying vein showed focal narrowing (mean relative narrowing: veins, 7%). Systemic treatment was instituted in 13 patients. The mean interval between FIAO imaging sessions was 3.1 months (range: 0.5- 24 months). Fluctuations of the width of sheathing and of venous diameters were documented. In 3 cases, it could be showed that thickness of sheathing temporally and spatially correlated with deformation of adjacent axons and vessels. Mean follow-up was 6.5 months (± 5.6). At last examination, there was reduction of sheathing (from 89.6 µm ± 45 to 33.1 µm± 15.6, p<0.03).
FIAO imaging is of interest to identify, quantify and monitor venous sheathing during the course of vasculitis. Fluctuation of the width of sheathing and of vessel narrowing is commonly observed, and may suggest activity of the disease. Focal variations of venous diameters at site of sheathing may be of compressive nature, which may provide insights into the cause of inflammatory vein occlusion.
