Abstract

Purpose : To evaluate the clinical significant change in central foveal thickness (CFT) measured 24 hours after intravitreal injection conbercept (IVC) for diabetic macular edema (DME).

Methods : Retrospective interventional case series. 23 eyes of 23 patients with DME who received monthly IVC (0.5 mg/0.05 mL) 3 times. CFT was measured using spectral-domain optical coherence tomography (sd-OCT) before injection, 24 hours and each month after injection until 3 months.

Results : Statistically significant CFT reduction was observed at every key time points. Before injection as baseline, 24 hours after injection, 1,2 and 3 months after first injection. CFT declined from 587.5±125.8 µm at baseline to 423.7±117.9 µm 24 hours after first injection (p<0.01) and to 337.4±101.4 µm at 3 month after first injection (p<0.01). The reduction in CFT 24 hours after injection was significantly correlated with the reduction in CFT observed at 3 months after 3 injections, with respect to both absolute (r = 0.614, p = 0.008) and relative changes (r = 0.521, p = 0.019). The 13 eyes that had larger 24-hour CFT reductions also showed larger 3 month CFT reductions (187.5±76.7 µm and 31.8%±12.3%) when compared with 10 eyes with smaller 24 hours CFT reductions (96.8±51.5 µm and 16.5%±9.8%). This trend was the same for absolute and relative changes (p = 0.011 and 0.018).

Conclusions : The 3 month response of DME to IVC can be predicted by checking CFT 24 hour after first injection.
