Abstract

Purpose : Optical coherence tomography angiography (OCT-A) facilitates qualitative and quantitative assessment of the microvasculature of the eye. The diagnostic accuracy of optic nerve head (ONH) vessel density measurements has been shown to be similar to retinal nerve fiber layer (RNFL) thickness for discriminating between glaucoma and healthy eyes, but the relationship between macular vessel density and ONH vessel density has not yet been described. The purpose of this study was to evaluate the strength of the associations between ONH and macular OCT-A vessel density measurements with standard structural OCT measures.

Methods : This cross-sectional study included 335 eyes of 335 participants from the Diagnostics Innovations in Glaucoma Study (DIGS). This included 221 primary open angle glaucoma (POAG) patients, 40 glaucoma suspects, and 74 normal participants with good quality Optovue AngioVue images according to the Shiley Eye Institute Imaging Data Evaluation and Analysis (IDEA) Reading Center. The strength of the association between microvasculature measures in the ONH (whole image vessel density in the retinal nerve fiber layer (RNFL) and the macula (superficial layer) was assessed using linear regression models. Associations between ONH and macular vessel density, and RNFL thickness and ganglion cell complex (GCC) measures were also evaluated.

Results : All relationships between each variable were highly significant (p <0.001). A positive association was found between ONH whole image and macular vessel densities (R2 = 39.1%). This association was weaker than the relationship between GCC and RNFL thickness (R2 = 69.8%). Both RNFL and GCC thicknesses were more strongly associated with the ONH vessel density (R2 = 36.8% and 25.6%, respectively) than macular vessel density (R2 = 24.2% and 14.6%, respectively).

Conclusions : The strength and pattern of the association of ONH and macular microvasculature dropout and structural loss show promise for improving detection of glaucomatous progression.
