Abstract

Purpose : We investigated the association between drusen and retinal thickness on standardized SD-OCT measurements obtained from participants enrolled in the Ontario Neurodegenerative Disease Research Initiative (ONDRI) study.

Methods : We evaluated the SD-OCT (Spectralis, Heidelberg Engineering, Heidelberg, Germany) images of one randomly selected eye from 137 participants with Parkinson's or Alzheimer's, diseases: 37 individuals with clinically visible drusen on SD-OCT and 100 without. Drusen height and width were measured in every cross-sectional macular scan contained within the 1, 2 and 3 mm diameters of the ETDRS grid centered on the fovea. Assuming a semi-elliptical shape for the druse, area and total volume of drusen were determined as the sum of all drusen within each of the 9 ETDRS grid sectors. HEYEX software (version 6.7.12.0) was used to batch-export average retinal thickness. An independent t-test was used to compare total retinal volume of those with and without drusen, and a linear mixed-effects regression model was used to compare total retinal thickness of those with and without drusen, accounting for within-subject variation. . For those with drusen we also performed a linear mixed-effects regression model to explore the association between retinal thickness and drusen volume (1) in the central macula and (2) in all other sectors. Statistical analyses were was performed with RStudio (version 1.0.136).

Results : There was no statistically significant difference in total retinal thickness or volume between those with and without drusen. For those with drusen, retinal thickness increased (ß=4.295, SE=0.927, p<0.01) with the natural logarithm of drusen volume; where a greater increase was seen in the central macular sector (ß=1.839, SE=0.933, p<0.01). Mean squared-error criterion identified a threshold (0.007 mm3) above which drusen volume was significantly associated with retinal thickness.

Conclusions : No difference was found in total retinal thickness or in total retinal volume between those with and without drusen. However in those with drusen, drusen volume was associated with an increase in retinal thickness where a greater increase was observed in the central macula compared to non-central macula sectors.
