To evaluate the short-term efficacy and safety of the second-generation trabecular meshwork (TM) bypass stent (iStent inject®) (Glaukos Corporation, Laguna Hills, CA) combined with phacoemulsification in patients with open-angle or normal-tension glaucoma.
A retrospective study of consecutive combined phacoemulsification and iStent inject® surgeries performed in Asian eyes in a single institution from July to November 2017. Data was collected from operative and post-operative clinical charts. Outcome measures included best corrected visual acuity (BCVA), intraocular pressure (IOP), number of topical glaucoma medications, and adverse outcomes reported at 1 day, 1 week and 1 month.
Data from 20 eyes were collected. Majority were male (n=17, 85.0%) and of Chinese ethnicity (n=20, 100.0%). 15 (75.0%) had open-angle and 5(25.0%) had normal-tension glaucoma. Average cup-disc ratio was 0.8 (range: 0.4 to 0.95). Average mean deviation on Humphrey Visual Field analysis was -14.69 ± 8.22 dB and mean pattern standard deviation was 8.80 ± 3.68 dB. Mean pre-operative IOP was 16.23 ± 5.50 mmHg and mean post-operative IOP at 1 month was 13.38 ± 3.50 mmHg, with a difference of 2.85 ± 4.61 mmHg (p = 0.046). The mean number of medications was reduced from 2.15 ± 1.21 pre-operatively to 0.62 ± 1.32 post-operatively (p>0.001). The mean number of iStents inserted in each surgery was 2.1 ± 0.85. 15 patients (75%) were medication-free at 1 month after surgery. No post-operative adverse events were reported within the follow-up period in our study.
Combined phacoemulsification and iStent inject® demonstrates early efficacy in reducing the number of glaucoma medications in Asian eyes with open-angle or normal-tension glaucoma.
