Abstract

Purpose : To document various morphological features in patients with different stages of Best's disease (BD) using multimodal imaging (MMI) and to correlate these features with the OCT angiographic (OCTA) findings.

Methods : Prospective observational case series was performed in 6 eyes of 3 patients who underwent complete clinical examination including colour fundus photo, SD- OCT, FAF, EOG , ERG and OCTA, and the OCTA features obtained at different stages of BD were analysed and compared to those obtained on MMI.

Results : Two males and 1 female with mean age 44.33 years (range 12-80) were included in the study. EOG was reduced in all three cases with a normal ERG.OCTA showed increase in intercapillary spaces between small vessels when compared to Normal individuals. FAZ was distorted in all the eyes in both superficial and deep plexus levels due to the underlying lesion. Superficial and deep vascular network appeared to be grossly normal in psedohypopyon stage (1 eye) which appeared to be distorted in vitelliruptive (4 eyes) and atrophic (1 eye) stages. The extent of the lesion was evident from the deep plexus. The details of the lesion were more pronounced on the Enface outer retina slab. Choroidal layer revealed bright and dark areas corresponding to the lesion on FAF and FFA. FAF and prearterial phases of FFA showed a good morphological correlation with the choroid slab of OCTA.

Conclusions : We conclude that OCTA along with enface provides insight in the pathological changes occuring at different retinal layers and has a very good correlation to the fundus autofluorescence.
