Visual field characteristic comparison between MS-ON and NMO-ON: Six months follow-up after acute onset

Abstract

Purpose : Through comparison of the visual field change of Multiple sclerosis related optic neuritis(MS-ON) and neuromyelitis optica related optic neuritis(NMO-ON), from onset to 6m after , to better understand the precise involvement in optic nerve.

Methods : Prospective, single-center, case-control study. 54 cases of MS-ON and 27 cases of NMO-ON were included. MD(mean defect) and PSD(pattern standard deviation) of VF at onset,1m,3m,6m after onset were analyzed.

Results : Results: For VF pattern: At the onset,diffuse abnormalities were the most common pattern in both group(P>0.05).One month after attack, central abnormalities began to arise in MS-ON(13%;P=0.031).The ratio of nerve fiber bundle abnormalities and diffuse abnormalities of the two groups was higher than the previous stage(22.9%,31.4% versus 22.5%,30%),of which 14.3% and 1.3% were altitudinal defcits in NMO-ON and MS-ON(P=0.015).The proportion of neurologic abnormalities in NMO-ON went up than before as well.Three quadrant abnormalities were found in 8.6% of the MS-ON eyes versus 0 of the NMO-ON eyes (P=0.027).Three months after attack, central abnormalities and normal VF was found in NMO-ON(8.6%,2.9%),while the other VF pattern had no obvious change between the two groups.Six months after attack,?% of normal visual field was found in MS-ON, while still no normal pattern was observed in the NMO-ON group(P=0.01).For VF MD & PSD: MD & PSD in the NMO-ON group was found higher than the MS-ON group,especially in 1 month and 6 to 12 months after attack(-18.7,-13.86 versus -11.9,-7.43;P=0.02,P=0.005).

Conclusions : Conclusion: VF recovery were quicker and better in MS-ON than NMO-ON. Diffuse damage was the most common in NMO-ON ,while Central and paracentral was the most common in MS-ON. Partial hemianopia was only found in NMO-ON. Altitudial heminopia were more frequent in NMO-ON, while arcuate defect was more frequent in MS-ON .
