Abstract
Purpose :
To develop new pediatric vision questionnaires designed to assess health-related quality of life (HRQOL) and functional vision.
Methods :
Previously, patient-derived master questionnaires were created from specific concerns identified from child and parent interviews. In the present study, these Child (n=97 items), Proxy (n= 97 items) and Parent (n=69 items) master questionnaires were administered to children (5-18 years old; n=278) and parents (n=444) across 10 diagnostic categories (esotropia, exotropia, hypertropia, amblyopia, refractive error, anterior segment, CNS, nystagmus, orbital, retina/optic nerve) to derive shorter, Rasch-calibrated questionnaires. Exploratory factor analyses were performed (for children aged 0-4, 5-11 and 12-17 years) to identify unidimensional domains. Rasch analysis was performed on each domain by age group (0-4, 5-11 and 12-17 years), to reduce to no more than 10 items per domain, evaluating category utilization, differential item functioning, response ordering, local dependence, fit, and targeting.
Results :
Five domains were identified for Child questionnaires (functional vision, bothered by eyes/vision, social, frustration/worry, glasses), 4 for Proxy 0-4 year olds (functional vision, bothered by eyes/vision, social, and glasses), and 6 for Proxy 5-11 and 12-17 year olds (functional vision, bothered by eyes/vision, social, frustration/worry, eye-care, and glasses). Five domains were identified for the Parent questionnaire (impact on parent/family, worry re: child’s eye condition, worry re: child’s self-perception and interactions, worry re: child’s visual function, and effect of treatment). Rasch analysis yielded a Child (5-11 year) questionnaire with 50 items across 5 domains, Child (12-17 year) questionnaire with 46 items across 5 domains, Proxy (0-4 year) questionnaire with 38 items across 4 domains, Proxy (5-11 year) questionnaire with 46 items across 6 domains, Proxy (12-17 year) questionnaire with 49 items across 6 domains, and a Parent questionnaire with 45 questions across 5 domains.
Conclusions :
By following a rigorous process for questionnaire development, we have created the new patient-derived Pediatric Vision Questionnaire (PVQ) with age-appropriate Child, Proxy, and Parent versions for assessing HRQOL and functional vision in childhood eye conditions.
