Abstract

Purpose : To evaluate the changes in the foveal avascular zone (FAZ) of superficial capillary plexus (SCP) and deep capillary plexus (DCP) and its association with visual acuity in eyes with idiopathic epiretinal membrane (iERM).

Methods : Twenty three eyes with unilateral iERM and postoperative follow-up duration of 6 months were included in this retrospective study. The changes in the best corrected visual acuity (BCVA, logMAR), FAZ area in SCP and DCP and central subfield macular thickness (CSMT) were reviewed. The area of FAZ was assessed using the swept source optical coherence tomography angiography (SS-OCTA, DRI OCT-1, TOPCON, Tokyo, Japan) as well as the ratio of FAZs in affected and unaffected eyes.

Results : The areas of FAZs in SCP and DCP showed a significant correlation with BCVA (r = -0.464, P = 0.026 and r = -0.702, P <0.001) and CSMT (r = -0.646, P = 0.001 and r = -0.606, P = 0.002) at baseline, but did not at follow-up visits. The ratio of FAZ in SCP and DCP was also correlated with BCVA (r=-0.719, P = 0.004 and r= -0.659, P = 0.010) and CSMT (r = -0.622, P = 0.018 and r = -0.702, P = 0.005) at baseline. Mean area of FAZ in SCP significantly increased (P =0.033) but did not in DCP after surgery. The change in FAZ of SCP was statistically correlated with the change in BCVA (r = -0.338, P = 0.034).

Conclusions : The smaller area of FAZs in SCP and DCP was observed in eyes with the worse visual acuity and thicker CSMT. The enlargement of FAZ in SCP, which resulted form the relaxation of tangential contraction in macula, was significantly associated with the visual improvement after iERM removal. Assessment of FAZ using OCTA can provide the additional information of the changes in foveal microstructure in eyes with iERM.
