Abstract

Purpose :
To study correlations in patients with sickle cell retinopathy (SCR) between peripheral retinal nonperfusion assessed using ultra-wide field fluorescein angiography (UWF-FA) and the automatically quantified macular vascular densities in the superficial (SCP) and deep capillary plexus (DCP) obtained using optical coherence tomography angiography (OCTA).

Methods :
Prospective, observational study of patients with sickle cell retinopathy who underwent a comprehensive ophthalmic examination including UWF-FA (Optos, Marlborough, MA) and OCTA using the AngioVue OCTA system (OptovueRTVue XR 100; AVANTI, Inc, Fremont, CA). Vascular densities in the superficial capillary plexus and DCP, as well as the area of the foveal avascular zone, were measured using the AngioAnalytics software.

Results :
Fifty-two eyes of 26 consecutive patients (15 men, 11 women) with SCR were included between June 2017 and November 2017. Mean age was 27 years, and 17 patients (34 eyes) were SS, 9 patients (18 eyes) were SC. Three patients were previously treated with peripheral retinal photocoagulation laser in both eyes (6 eyes). Mean best-corrected visual acuity (BCVA) was 0,03 (LogMAR). Peripheral retinal ischemia was found in 39 eyes (75%) on UWF-FA. Our study showed a tendency toward correlation between peripheral nonperfusion on UWF-FA and automatically quantified global vascular density in both plexus (P = 0.05 for the SCP).

Conclusions :
Our study demonstrated a tendency toward correlation between peripheral nonperfusion on UWF-FA and automatically quantified macular vascular density on OCTA; OCTA could help identify high-risk sickle cell retinopathy patients who may benefit from further evaluation using UWF-FA. UWF-FA allowed a more precise topographic analysis of vascular abnormalities and ischemia areas than with conventionnal FA.
