Abstract

Purpose : To determine the long-term results of photodynamic therapy with verteporfin (PDT) for a serous retinal detachment (SRD) associated with a circumscribed choroidal hemangioma (CCH).

Methods : Five eyes of 5 patients with a serous retinal detachment secondary to a CCH were studied. Three patients had a peripapillary CCH, one patient had a CCH involving the macular, and the other had a CCH located just temporal to the macular area. All patients underwent PDT over the entire tumor. The PDT was performed by multiple laser exposures for the peripapillary CCH to avoid damages to the optic nerve head. The laser spots were located >200 μm from the optic nerve head. Single or multiple laser spots were applied for the macular CCH or the CCH located at the extra macular area. All patients were observed for at least 3 years after the initial PDT treatment.

Results : The mean age was 49.6 years (29-62 years), and the mean follow-up period was 74.2 months (52-110 months). The mean best-corrected visual acuity (BCVA) increased from 0.39±0.50 to 0.30±0.48 logMAR units. All cases had a complete regression or a decrease of the SRD within 12 months. One patient required 3 application of PDT to reduce the activity of the CCH. Two patients required retreatments for recurrences of the SRD at about 53 months after the initial PDT. All patients had a complete regression of SRD, but a cystoid macular edema remained in 2 patients.

Conclusions : The results indicate that PDT is an effective procedure to reduce the SRD secondary to CCH for at least three years. Two cases had a recurrence of the SRD because of the reactivation of the CCH about 53 months after the initial PDT but PDT was effective in resolving the recurrences.
