Abstract

Purpose : To assess iridocorneal angle by AS-OCT using a novel methodology and to compare assessment with clinical gonioscopy

Methods : Observational case series of patients with glaucoma or ocular hypertension. Iridocorneal angle images and measurements were made using images obtained from Heidelberg AS-OCT. The Schwalbe’s line was identified and a circle was drawn between the Schwalbe’s line and the iris. Diameter of the circle was used to approximate the iridocorneal angle width. Clinical gonioscopy images were also taken from the same location of the AS-OCT images and angles were graded based on Shaffer grading system and compared to the corresponding AS-OCT images for each eye.

Results : Thirty eyes were analyzed in this study. The average angle widths measured by AS-OCT were 166±42µm for Shaffer Grade 1, 349±49µm for Shaffer Grade 2, 447±106µm for Shaffer Grade 3, 563±128µm for Shaffer Grade 4. A positive correlation was found between angle width and clinical Shaffer grade (Pearson: 0.8, p<0.001).

Conclusions : In this study, angle width was measured by AS-OCT. A strong agreement was observed between measurements made from clinical gonioscopy and AS-OCT. AS-OCT also provides additional information that can be used in conjunctional with gonioscopy for iridocorneal angle assessments. Further studies with larger sample size will be needed to determine the validity and utility of implementing this method into clinical practice.
