Abstract

Purpose : Laser iridotomy (LI) has been long used to treat eyes with primary angle closure glaucoma (PACG) or its related pathologies. However, several studies suggested deleterious effects of LI on corneal endothelial cells (CECs) and only 3-year post-LI effects on CECs were available in the literature. In this study, changes in the CEC density (CECD), central anterior chamber depth (CACD), and gonioscopic findings of LI-treated and -untreated eyes were followed for 5 years.

Methods : One hundred and thirteen newly LI-treated and 145 LI-untreated primary angle closure (PAC) or PAC-suspect (PAC) eyes of 113 and 145 subjects, respectively, were followed for 5 years with CECD, ultrasound biomicroscopic (UBM) and gonioscopic examinations every 12 months.

Results : Sixty three newly LI-treated eyes of 63 subjects completed 5-year follow-up without changing medications and 61 eyes of 61 subjects without LI (LI-untreated eyes). No significant inter-group differences were seen in the systemic or ocular factors except for treatment with LI with average energy of 806.9 ±547.2 mJ (standard deviation) mJ and a small difference in the age. No difference in the CECD change (P>0.20) was seen between the LI-treated (2648±299/mm2 at baseline before LI and 2623±303/mm2 at 5 years) and -untreated eyes (2579±326/mm2 at baseline and 2604±360/mm2 at 5 years), while CACD similarly decreased in the LI-treated (2.54±0.23mm at baseline before LI and 2.47±0.22mm at 5 year, P<0.001) and -untreated eyes (2.56±0.21mm at baseline and 2.51±0.23mm at 5-year, P=0.006). Goniscopic Shaffer grades increased in all quadrants in LI-treated eyes (P=0.018), while they decreased in all quadrants in the LI-untreated eyes (P< 0.001).

Conclusions : LI showed no significant effects on the CECD during the 5-year follow-up, while it significantly increased angle width.
