Abstract
Purpose :
To observe retinal arterial macroaneurysms (RAMs) using optical coherence tomography angiography (OCTA) and try to classify them by their morphology.
Methods :
Nineteen consecutive eyes with RAMs were studied. OCT angiograms, 3 mm×3 mm area centered on the RAM, were obtained with the RTVue-XR Avanti (Optovue). The morphology of RAMs was classified into a saccular type, a fusiform type, or an unclassified, according to the classifications of cerebral or thoracic aneurysms. In ruptured cases, the vessel structure was also evaluated with en-face images, regardless of flow signals in the vessel.
Results :
In 19 cases, 7 cases (37%) were saccular type, 4 cases (21%) were fusiform type, and 8 cases (48%) were unclassified. Ruptured RAMs were observed in 12 cases (63%); saccular type in 5 (42%), fusiform type in 4 (33%) and unclassified in 3 (25%). En-face images revealed Z-shaped vessels, which were previously described in the study with fluorescein angiography (FA), in 3cases with resolved RAMs, in which the type was saccular and the Z-shaped vessels were formed by three-dimensionally bending at the RAM site.
Conclusions :
The morphology of RAMs could easily observed and classified with OCTA without any side effects such as anaphylaxis, a possibly lethal complication of FA.
