Abstract

Purpose : We measured the improvement in vision and vision function after bilateral cataract surgery, and evaluated the correlation between change in vision and vision function.

Methods : We collected Best Corrected and Habitual Snellen Visual Acuity (BCVA and HVA) and Vision Function (Catquest 9SF questionnaire) up to 6 months before the first surgery, and 4-12 weeks after the second surgery. HVA was defined as visual acuity with their current spectacles. BCVA ‘worse eye’ was defined as the eye with the poorer visual acuity pre-operatively. The possible range in Catquest scores was -3.94 (no difficulties) to +3.52 (great difficulties). Data was collected electronically at four sites in Ontario, Canada (Kensington Eye Institute, Prism Eye Institute, Peterborough Regional Health Centre and Ottawa Eye Institute). It was directly uploaded to a web-based case report form (REDCap™) and analyzed at the Applied Health Research Centre of St Michael’s Hospital. The Catquest 9SF questionnaire was scored using a pre-programmed spreadsheet provided by the developer (M Lundstrom, Sweden).

Results : 190 subjects were enrolled. We collected post-operative visual acuities on 166 and Catquest 9SF data on 150. The average age was 70 (SD 8.5), and 57% were female. 14.2% had comorbidity affecting their central vision. Mean pre- operative BCVA ‘worse eye’, was 20/55. Mean pre- operative Catquest 9SF score was -1.04 (SD 1.76). After surgery, there was a statistically (p<0.00001) and clinically significant improvement in BCVA ‘worse eye’ of -0.37logMAR (95%CI -0.42, -0.31). The Catquest 9SF score improved by -2.56 (95%CI -2.85, -2.27). The correlation between change in HVAOU and change in Catquest 9SF was moderate (r2=0.485).

Conclusions : BCVA ‘worse eye’ which is often the indicator used for recommending cataract surgery, improved by almost 4 lines. Vision function with the Catquest 9SF improved significantly, and the correlation between change in HVAOU and Catquest score was moderate. The Catquest 9SF is an additional metric that can be used to assess outcomes of cataract surgery.
