Abstract

Purpose : Selective laser trabeculoplasty (SLT) is a commonly used office procedure to reduce intraocular pressure (IOP) in patients with glaucoma, but the effect of SLT decreases with time. The purpose of this study was to determine outcomes of repeat SLT in patients with open-angle glaucoma.

Methods : A retrospective chart review of all patients who underwent SLT by 3 glaucoma specialists (CS, MRM, MJP) at Wills Eye Glaucoma from 2014 to 2018 was performed. All patients were diagnosed with open-angle glaucoma and had follow-up of at least 3 months. The repeat SLT group included any patient who had a second SLT treatment without any additional intervention between the two SLT treatments.

Results : A total of 887 eyes (796 primary open-angle glaucoma, 69 pseudoexfoliative glaucoma, 22 pigmentary glaucoma) were included. The mean age was 73.4 ± 11.6 years with 488 (55%) females. For the initial SLT, the mean baseline IOP was 19.8 ± 5.2 mmHg on 2.0 ± 1.3 IOP-lowering medications. The mean follow-up time was 9.3 ± 3.5 months. The mean IOP at the most recent follow-up visit was 16.1 ± 4.5 mmHg (P<0.001) on 1.8 ± 1.3 medications (P<0.001). SLT treatments ranged from 270 to 360 degrees with a mean power of 61.9 ± 25.1 mJ. Fifty-four eyes underwent repeat SLT treatment. The mean time to repeat SLT from initial SLT was 2.4 ± 1.3 years. The mean IOP prior to repeat SLT was 20.1 ± 4.4 mmHg on 2.2 ± 1.2 medications. The mean follow-up time after repeat SLT was 1.7 ± 1.0 years. The mean IOP at the most recent follow-up visit after repeat SLT was 17.1 ± 4.0 mmHg (P<0.001) on 2.1 ± 1.4 medications (P=0.46). There were no post-SLT IOP spikes after the second SLT treatment.

Conclusions : Repeat SLT is an effective treatment for the reduction of IOP in patients with open-angle glaucoma.
