OCT Angiography represents a paradigm shift of our ability to clinically investigate the retinal microvasculature. It provides immediate non-invasive access to capillary beds allowing a clearer understanding of macular and optic nerve pathologies which accompany or precede retinal structural changes. Beyond qualitative anatomic imaging, it offers the unique capability of quantitative analysis for measuring subtle progression of disease, as well as the metabolic impact of traumatic events and therapeutic interventions. Examples of its use in investigating the perfusion impact of acute intraocular pressure events, neurologic episodes, and therapeutic interventions will be discussed. Strategies for identifying early pathological changes from normal variations in perfusion will also be presented.
