Abstract

Purpose : To evaluate the changes of the choroidal structure in Vogt-Koyanagi-Harada (VKH) patients with or without recurrence of inflammation when choroidal thickening occurred during the treatment.

Methods :
In this retrospective study, all consecutive 20 eyes of 20 patients with acute VKH (15 females, mean age 56.0±8.9 years) who were followed over 12months were enrolled. Five patients had recurrence of inflammation (recurrence group) and 15 patients had no recurrence of inflammation (no recurrence group).All patients were examined by enhanced depth imaging optical coherence tomography (EDI-OCT) at every visit. We defined an increase of more than 100 µm in subfoveal choroidal thickness (SFCT) compared to that one month before as choroidal thickening. The primary outcome measures were the differences of choroidal intensity (CI) and choroidal stromal ratio (CSR) with the Niblack binarization method from EDI-OCT images between at the time of choroidal thickening and at one month before.

Results : Choroidal thickening occurred in all of 5 patients with recurrence of inflammation, occurred in 5 out of 15 patients without recurrence (p=0.03, Fisher’s exact test). The total choroidal thickening are 8 times in recurrence group and 7 times in no recurrence group. In recurrence group, mean SFCT (µm) significantly increased from 267 ± 74 to 464 ± 116 (P<0.001, paired t test). Mean CI significantly decreased from 0.44 ± 0.21 to 0.33 ± 0.13 (P=0.03) and CSR decreased from 0.44 ± 0.21 to 0.33 ± 0.13 (P=0.08). In no recurrence group, mean SFCT (µm) significantly increased from 236 ± 74 to 404 ± 103 (P<0.01). On the other hand, mean CI (from 0.51 ± 0.14 to 0.48 ± 0.20) and CSR (rom 0.19 ± 0.04 to 0.19 ± 0.08) had no significant change.

Conclusions :
Mean CI and CSR in recurrence group decreased when choroidal thickening occurred, but they had no significant change in no recurrence group. The changes of CI and CSR in choroidal thickening might relate to recurrence of inflammation in VKH.
